The first one is for general information, and we complete it together. The second one is focused on the <a href=\"https:\/\/library.samhsa.gov\/product\/creating-healthier-life-step-step-guide-wellness\/sma16-4958\">8 dimensions of wellness,<\/a> and can be completed together or by yourself. These two questionnaires lay the groundwork for your wellness plan.<\/p>\n<hr \/>\n<h3>What happens after the intake?<\/h3>\n<p>After the intake, our team provides your information to you in the form of a wellness plan, in whichever format you prefer.
