Wellness Coordination

Our Wellness Coordination service is a proactive, holistic approach that synchronizes care across multiple providers to prevent gaps, duplication, or confusion while simultaneously guiding Women through the steps toward improved physical, mental, and social well-being.

This is our main case management service for women and it has three areas of support. Each individual’s service outcome varies on upon available resources and on a case by case basis.
If we cannot be of service to you, we will not waste your time.


Information & Referral + Warm Handoff:

  • Basics! Before services can begin, we must complete an Intake & create your Wellness Plan.
  • Referrals + Warm Handoffs – these are two things that will be ongoing as we discover what areas you want to grow in.

->  A referral is simply suggesting a service, often just giving contact information. A warm handoff actively connects you to that service in real-time, with an introduction and shared information, ensuring the connection is made and reducing barriers to care… unlike referrals where you’re left to do all the work!


Prevention/Recovery/After Care Education:

  • We offer a variety of printed or online educational materials related to substance use prevention, recovery and after care. On our website you’ll find a Resources page that is available 24/7.
  • Our team provides Presentations and Workshops to help women gain knowledge and strengthen skills.
  • We host Alternative Activities and Events to provide fun, safe things for our women and families to do around our beautiful city!

Basic Necessity Items:

  • We provide our Women with much needed Basic Necessity Items such as: Hygiene Items, Baby Items, Household Items, Linens and more.
